Insurance & Reimbursement

A cash-pay practice, made simple.

Jujube Acupuncture and Herbal Medicine is a cash-pay practice and does not bill insurance directly. We are considered an out-of-network provider.

If your insurance plan includes out-of-network acupuncture benefits, we are happy to provide you with a superbill upon request. A superbill is an itemized receipt that includes the information typically required by insurance companies, including diagnosis and procedure codes, treatment dates, provider information, and the amount paid.

You may submit your superbill directly to your insurance company for possible reimbursement. Coverage and reimbursement vary by plan and are not guaranteed. We recommend contacting your insurance provider before beginning treatment to confirm your out-of-network acupuncture benefits, deductible, and reimbursement requirements.

HSA & FSA Payments

Acupuncture may qualify as an eligible medical expense for Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA). You may use an HSA/FSA card at the time of payment when accepted, or request an itemized receipt or superbill to submit to your account administrator.

Eligibility and documentation requirements are determined by your individual plan or account administrator.

How to Submit a Superbill

After your visit, request a superbill from Jujube. You can then submit it directly to your insurance company according to their out-of-network claim process. Most insurers allow claims to be submitted through their online member portal or with an out-of-network claim form.

Any reimbursement will be issued according to the terms of your individual insurance plan. Payment is due to Jujube in full at the time of service regardless of insurance reimbursement.

Jujube does not verify insurance benefits, submit claims on behalf of patients, communicate with insurance companies regarding claim processing, or guarantee reimbursement.

For questions about your specific benefits, deductible, reimbursement rate, or claim status, please contact your insurance company directly.

Cancellation Policy

Your appointment time is reserved especially for you. If you need to cancel or reschedule, please provide at least 24 hours' notice so that the appointment may be offered to another patient.

Appointments changed or canceled with less than 24 hours' notice will be charged 50% of the scheduled service fee. Missed appointments without notice will be charged the full service fee.
